Role Description
This is a contract remote role for an Operation Intern. The Operation Intern will be responsible for supporting daily operations, assisting in project management, and conducting analysis. Tasks will include collaborating with team members, managing schedules, tracking progress, and contributing to sales initiatives. The intern will also participate in strategic planning and operational improvement activities.

Job Summary Band 8A (Subject to Evaluation) Dartford, Gravesham and Swanley (DGS) Health and Care Partnership (HCP) is a partnership of NHS organisations, councils, voluntary care sector representatives, public health and local authority organisations. The HCP representatives work collaboratively to design and deliver better integrated services to meet the needs of the population they serve, with a particular focus on areas of greatest need. The purpose of this role is to support the embedding of a Population Health Management (PHM) approach across PCNs and wider stakeholders across the DGS footprint with the aim of reducing Health Inequalities (HI). The post holder will also be required to work closely with Public Health, local Council colleagues and the voluntary sector, to implement community engagement activities, developing local initiatives that help support our most vulnerable population. This role is one of four PHM Programme Manager posts, each based within one of Kent and Medway's Health and Care Partnership areas: Dartford, Gravesham and Swanley (DGS); East Kent; Medway & Swale and West Kent. There will be an expectation to work collaboratively with those personnel as well as link with the relevant Kent & Medway (K&M) ICB leads. Addressing health inequalities through PHM will be a key focus of the work. Main duties of the job Using a data driven approach, identify and tackle key areas of health inequalities within DGS, applying population health management techniques to priority areas. Operationally develop, lead and facilitate the Health Inequalities & Population Health Management programme and associated projects, including all relevant programme management and delivery plan documentation and reporting. Ensure appropriate management of programme objectives and outcomes, identifying and managing risks, issues and dependencies, considering best practice and making decisions in the best interest of the programme. Responsible for implementing quality improvements and changes locally at a DGS HCP level through collaboration with all relevant system partners. Operate effectively in a flexible and demanding environment and proactively engage with staff across all key stakeholders within the HCP within DGS. Present highly complex information about projects, initiatives and services to a wide range of stakeholders in a variety of formal and informal settings through written and verbal reporting.
